Payment and Insurance
Insurance
We absolutely take insurance. What your insurance will cover is based on your individual plan. If you have questions, please don’t hesitate to text or call us at 205-772-9595 and we can advise you further. We will need you to fill out our intake forms with photos of the front and back of your insurance card so that we can look into your coverage. If you prefer to pay yourself, our cash prices are below.
Medicaid
We take Medicaid for our nausea and vomiting patients as well as for iron infusions. If you have Medicaid and there are medicines that we prescribe for you that Medicaid doesn’t cover, we sell them for a fraction of the cost of CVS in our clinic. There is no copay for our Medicaid patients. Be aware you have Medicaid PLUS another plan that we are out of network with, Medicaid will not cover your visit to our clinic. Make sure to let us know about ALL insurance policies you have so that we can give you the best estimate of what your care might cost. If Medicaid won’t cover your care, you may qualify for financial assistance through our nonprofit, The HG Relief Project.
Blue Cross Blue Shield of Alabama
We are in network with Blue Cross Blue Shield* of Alabama for our nausea and vomiting patients as well as for iron infusions. PPO plans do not require referrals for our services. If you have BCBS HMO coverage, you may need a referral from your primary care physician or OB/GYN. If your plan begins with PGX, you will have to get a referral from your primary care physician to be seen in our clinic.
*We are NOT in network with the Blue High Performance Network plans. BCBS will not pay for any part of your visit if you are on this plan. If your policy starts with NDJ, you likely have this plan. We have tried to get in network with the High Performance Plan. If you have this insurance, call and request for Dr Housholder to be added! If you have the BCBS High Performance Network plan, may qualify for financial assistance through our nonprofit, The HG Relief Project.
Other Insurance Carriers
We are in network with Aetna, Cigna, Tricare, United Health Care* and VIVA. This means that you will be charged in network prices, and your individual policy and deductible determines what will be paid out of pocket for your appointment.
*Note: We are not able to accept applicants with United Healthcare- Alabama Exchange (ALONEX) / UHC HMO policies at this time due to out of network status. If you have one of these UHC plans, you may qualify for financial assistance through our nonprofit, The HG Relief Project.
*We do not accept Multiplan insurance plans, if you have Multiplan you may qualify for financial assistance through our nonprofit, The HG Relief Project.
